Shirley was born in Lafayette, Louisiana to Percy Sr. & Bernice
Guillory. Shirley was one of 5 children. She attended lake Charles High School and
went on to attend Demon Beauty School and later became a Certified Nursing
Assistant.

Shirley lived her life to the fullest & loved the lord. She enjoyed
puzzle books, spending time with her family & enjoyed being the neighborhood
mom.

Shirley is survived by her parents Percy Sr. & Bernice Guillory, of
lake Charles, LA. Percy Guillory Jr.(The late Beatrice), Danny Guillory Sr. ( Joanna),
Terry Simon (Patrick). Her husband Levi Hazeley III, Her children Levi IV,(Jodi}
Christopher(Tammy}, Shaun (Tracee), She also raised her grandson Levi Hazeley
Goodwin. She has 3 other Grandchildren Including her Great grandson Dawson
Curtis Goodwin, a host of Aunts, Uncles, Nieces, Nephews, Cousins, & Friends.

She was preceded in death by her sister Mary Alice Guillory, and
her partner Barron Campbell. Her Father in law Levi Hazeley II, & Mother in law
Muggie Hazeley.
